The cement truck and silver SUV collided on the northbound lanes of the Hume Motorway near Menangle Road at Menangle Park this morning.
The female driver of the SUV and a child died at the scene.
A baby was trapped in the car and had to be extricated before being treated by paramedics at the scene.
"Paramedics and two specialist medical teams worked alongside Fire and Rescue NSW to free the child who was then intubated on scene before being airlifted," NSW Ambulance Chief Superintendent Mark Gibbs said.
The baby was taken to Westmead Children's Hospital in a critical condition.
Gibbs said it was a miracle the baby survived the horror incident.
"I think it is hard to believe that anybody did survive that wreckage (from) such a tragic and high-speed incident," he said.
The truck driver was not injured and has been taken for mandatory testing.
Police are investigating the cause of the crash and will prepare a report for the Coroner.
The Hume Motorway remains closed in both directions at Menangle Park.
"Motorists are advised to avoid the area," Live Traffic NSW said.
"All northbound motorists are being diverted off the motorway at Picton Road and should consider using the M1 Princes Motorway or Appin Road instead.
"Southbound motorists are being diverted at Narellan Road where they can use Appin Road to travel south."
Live Traffic NSW said traffic is very heavy in both directions and drivers in the area should allow extra travel time.
