The young boy smashed the front window of a two-storey townhouse to escape the fire and was assisted by pedestrian walking past on Rodley Avenue in Penrith.
The home was full-engulfed when emergency services arrived on the scene shortly before 6.15pm.
The boy was treated by NSW Ambulance paramedics for lacerations before he was taken to Nepean Hospital for further treatment.
Police believe the child was the only person in the home at the time the fire broke out, with the child's mother arriving on scene a short time later.
A child-at-risk notification will be submitted, police say.
Fire and Rescue NSW firefighter Anthony Sibary said the home was completely destroyed by the blaze.
"There were no other damage to the adjoining properties," Sibary said.
Police said initial inquiries indicate the fire was not suspicious.
Fire investigators and scene-of-crime officers are working to determine if a candle left alight in the kitchen was the cause.
