Police are advising of extensive delays as Blaxland Road and First Avenue in Eastwood remain closed after the crash about 1.30pm.
"Two people were taken to hospital in critical conditions along with four others who sustained minor injuries," police said in a statement.
"Officers from Ryde Police Area Command have established a crime scene which will be examined by specialist officers from the Metropolitan Crash Investigation Unit."
The two people in hospital will undergo mandatory blood and urine testing.
"The road remains closed with local traffic diversions in place," police said.
"Extended delays are expected, and motorists are advised to avoid the area."
An investigation into the circumstances surrounding the crash is underway.
