The incident occurred about 7.45pm, with emergency services quickly dispatching a helicopter to Billyard Avenue in Elizabeth Bay.
Lauren O'Neill suffered significant injuries to her leg and blood loss.
A nearby veterinarian applied a tourniquet to stem the bleeding before the 29-year-old - who is a keen kayaker and is believed to have recently purchased a unit in the area - was rushed to St Vincent's Hospital in a critical condition.
O'Neill's injuries are believed to be severe to her lower right leg.
'Horrifying scene' for bystanders
Michael Porter told Today he had just gotten home from work when he heard screaming outside.
"I looked outside and Lauren was sort of pulling herself off the side of the harbour and (was) trying to get to (safety)," he said.
"Her leg was trailing behind her and the water behind her was all red with blood.
"We immediately sprinted outside, calling triple-zero.
"My neighbour was on the front of the steps as well and we both went straight over to her and sat by her side."

Nearby resident Georgia witnessed the incident said her wife was the vet who potentially saved the victim's life.
"We heard a shout and she swam up to a boat, but on the way back she got bitten by a bull shark," she told 9News.
"My wife's a vet and she basically bandaged it up… it's pretty hectic actually, but she seems to be OK, she's been in a lot of shock and so am I.
"We got some bandages, we kept her warm, and my wife basically bandaged her up to stop the bleeding."
She added that had the 29-year-old been further out in the water, "she wouldn't have survived".
Porter described his neighbour as an "absolute hero".
"I think she saved her life. She got straight into emergency mode," he said.
"People were holding her hand and helping her. (Lauren) was extremely brave the whole time and she was very lucid.
"She was talking and thanking us for getting the ambulance."
Victim stable after attack
NSW Ambulance Acting Inspector Brett Simpson said multiple paramedics and intensive care teams including an ambulance helicopter carrying blood transfusion products rushed to the scene.
"On arrival, our paramedics found her on a private wharf with significant lower limb injuries and a large amount of blood loss," he said.
"We believe a young lady was swimming off a private wharf and was assisted by bystanders.
"She was taken in a serious but stable condition to St Vincent's Hospital."
Inspector Simpson praised bystanders for their quick thinking to treat the 29-year-old.
"The bystanders did an exceptional job looking after this young lady, as did our paramedics by stabilising her," he said.
She was taken to hospital where she remains in a stable condition.
City of Sydney Councillor Linda Scott warned residents to keep out of the harbour.
"I'm extremely concerned to learn of a shark attack in our City of Sydney this evening," she said on Facebook.
"A young women has received severe injuries as a result of tonight's shark attack in Elizabeth Bay, west of the Marina, near the Navase.
"Please, stay out of the harbour until further notice."
The last fatal shark attack in Sydney was back in 2022.
Diving instructor Simon Nellist, 35, was killed after being mauled by a shark off Little Bay.
